El Dorado Urgent Care employs a simple and easily completed registration process. To ensure accuracy and speed the completion of your registration, please bring the following items with you:

Insurance Card

Photo I.D.

List of current prescription and non-prescription medications.

List of any allergies you may have.

For your convenience, click here to print our registration form. For best results when printing, view the registration form using Windows Internet Explorer. You can complete it at home and present it to the receptionist upon arrival.
